• Emergency Dental Treatment Notice and Acknowledgment of Risk - An informational form on the risk of COVID-19 during emergency dental treatment.

• Patient Disclosures - Form for patient to disclose if they have any risk attributes for the COVID-19 virus which may impact treatment decisions. The Provider should follow up on any affirmative answers to the questions posed on this form with the patient prior to treatment and document the discussion

COVID-19 Information for Registered Sponsors – On Sunday, March 22, 2020, Governor Carney ordered non-essential businesses in Delaware to close. The orders went into effect at 8:00 a.m. on Tuesday, March 24, 2020. They will remain in effect until May 15 or until the public health threat is eliminated.
